Long Term Care Binder ...because good information management enables better care.

Developed by caregivers for caregivers, the 28-page Personal Care Binder helps caregivers — both professional and family — keep track of all those critical details needed when co-ordinating care for an a loved one or client at home. The binder helps you stay organized, keeping much-needed personal and medical information available for quick access. It helps centralize communication among all the caregivers by providing a place for the care team to leave important messages for each other.

A new section has been added for those involved in dementia care.
